Single seniors having incomes equal to or under $19,300 and senior couples with combined incomes equal to or under $32,300 apply for this service. Such customers will not pay annual deductible, along with co-payments of as much as $2 per prescription. This will result in savings of about $130 each year from out-of-pocket drug costs. In order to be eligible all you have to do is fill out a co-payment application form for seniors, sending it to the Ministry of Health and Long-Term Care.
